FBP, Inc. will accept new applications for association membership and group insurance coverage from PA licensed and properly insured brokers and agents. To be appointed by FBP, Inc. we will require

• Current valid PA Insurance License
• Current E & O Face Page
• W-9

The county in which your clients business is domiciled will determine which association and group insurance plans, they are eligible to join. The chart below provides guidance on county of business domicile and its corresponding assocation.
