1. W-9 Form.
2. Copy of your current Pennsylvania Insurance license.
3. Copy of your Errors & Omissions Insurance

In order to become an IBC Appointed Agent you must supply Flexible Benefits Plans with the following information through email or fax (610-482-1803):

1. IBC Producer Schedule E.
2. IBC New Producer Form
