Effective April 1 2008, the Account Installation New Business Department will require the following information for Licensed Professionals: 

• New Business Paperwork
• Professional License
• Proof of Income (Schedule C)
  

 This IBC Underwriting change goes into effect with all April 2008 New Business submissions.
