Independence Blue Cross has released the following information in an eBulletin on March 19, 2010.

Connections^SM Accordant Care^TM Program for complex conditions transitions to IBC's Care Management team

IBC's disease management program for complex chronic conditions has been provided in collaboration with an external vendor, Accordant Health Services, since 2005. We have enhanced our internal care management capabilities and now have the expertise to provide these services using our own staff.

Therefore, as of May 1, 2010, we are transitioning the services for members in fully-insured groups who have these conditions to an in-sourced model. This same transition will occur by December 31, 2010 for members in self-funded groups who have purchased the Connections program.

Communicating to members
Later this month, we will send a letter to all members affected by this transition notifying them of the change and providing additional information on available resources and tools. In addition, we will reach out by telephone to members who are actively engaged with a coach to introduce ourselves and discuss this change with them.
