October 2007 Broker Notes

Open Enrollment Nears!

FBP has been working hard to ensure that this year's Open Enrollment is a smooth one. Please review the Key Dates and Notes on the reverse side for details.

Association Transfers

Once again this year, FBP is accepting Association of Record (AOR) letters to transfer Association business to Flexible Benefits Plans. The letters must be completed by the transferring group and returned on company letterhead to FBP by Friday, February 8, 2008. This letter will allow IBC to forward the renewal rates to FBP for an April 1, 2008 effective date.

All transfers must then re-enroll through FBP by Friday, February 8, 2008. Transfer packets must include:

·                  UC-2A for group re-qualification

·                  Benefit Election form

·                  Renewal rates for selected option

·                  Premium check, payable to FBP

·                  Member Applications

·                  Waivers (if applicable)

·                  Copy of BOR/AOR with updated date (if applicable)

Upgrades, Downgrades

All groups requesting coverage changes must complete the benefit change form and submit it by Wednesday, November 21, 2007 to get a January 1, 2008 change effective date. This form must also be signed by a company official.

FBP Key Dates and Notes- Fall 2007

10/25/2007

Tentative renewal rates e-mailed to brokers
These rates should not be released to employer groups until October 26th to allow IBC time to revise the rates, if necessary.

10/26/2007

Production of renewal letters begins
Letters will begin to be produced and mailed beginning October 26th. Letters will be staggered to assist you in managing your customer base.

11/21/2007

All paperwork for coverage changes due
All groups requesting coverage changes must complete the renewal form or a benefit change form and submit it by Wednesday, November 21, 2007 to get a January 1, 2008 change effective date. A company official must sign this form.

Groups changing lines of business must complete a Universal Enrollment Form.

12/10/2007

New Business paperwork due
All paperwork must be received at FBP to qualify for a January 1, 2008 effective date.

2/8/2008

Association of Record transfer letters due
Letters must be completed by the transferring group and returned to FBP by Friday, February 8, 2008 to qualify for a April 1, 2008 transfer date.
