April 2007 Broker Notes

In recent weeks, the Broker Community has expressed an interest in the New Individual HMO being offered by Keystone Health Plan East.

Flexible Benefits Plans, Inc. is pleased to be able to offer this coverage as part of our Independence Blue Cross / Keystone Health Plan East product portfolio. The following is a brief overview on how to apply for this coverage:

• Contact our Underwriting Department at 610-482-1800 or visit www.flexiben.com to obtain KHPE Individual HMO Enrollment and Sales Materials.

• Complete the Member Application online by logging on to www.flexiben.com or Independence Blue Cross

  Whether applying online or using a paper member application, be sure to complete all sections of the application and provide medical information on all persons applying for coverage. Primary applicant must be the oldest person applying for coverage.

• Include the first premium payment. If paying by check, the check should be made payable to Keystone Health Plan East, represent the first three months of premium and be included with all of the paperwork submitted to FBP. (For example, if your estimated premium is $196.00 a month, your check should be in the amount of $588.00)

  If the member elects to pay electronically by ACH (Automated Clearing House), they must authorize withdrawal of payment from their bank account. The amount withdrawn is the invoice total amount due. In addition to the monthly premium, the total amount due may include retroactive premiums due to incorrect bank information or returns as a result of insufficient bank account funds.

• If ACH is elected as the payment method, an ACH Authorization Form is required. This form is available online and within the Enrollment Kit.

• Regardless of the Enrollment Method, please forward all the member paperwork to Flexible Benefits Plans, Inc. along with the Producing Agent Information Sheet.
